Ma. Del Refugio's father was Pablo Macias and her mother was Ma. Del Refugio Martinez (Macias).  Her paternal grandparents were Quirino Macias and Ma. Del Refugio Martinez; her maternal grandparents were Eulogio Martinez and Jacinta Aranda. She had two brothers and four sisters, named Florencio, Crecenciano, Delfina, Ma. Cleofas, Sebastiana and Agapita.  She was the oldest of the seven children. She had a half-brother and two half-sisters, named Anastasio, Lina and Virginia.
